DOT Physicals

A Department of Transportation (DOT) physical examination must be conducted by a licensed medical examiner listed on the Federal Motor Carrier Safety Administration (FMCSA) National Registry. At Midwest Express Clinic, our providers are registered with the FMCSA to perform these examinations.

If the medical examiner determines that the individual is physically qualified to drive a commercial motor vehicle (CMV), they will provide a copy of the results to the individual and complete a Medical Examiner’s Certificate.

To ensure a quick and efficient visit, please fill out the DOT Consent Form prior to your appointment. This form will help you gather all the necessary paperwork for the medical examination. If your primary care provider needs to complete the 5870 paperwork, you can download below.

Remember, this is the medical examination portion required for your license. You will still need to take the completed paperwork to the Bureau of Motor Vehicles (BMV) to apply for licensure.
