How Self-Pay Works at Midwest Express Clinic
Self-pay means paying for your visit directly, without billing an insurance company. We accept cash, debit, and credit cards at every location. We also partner with CareCredit, a healthcare financing option that lets qualifying patients spread the cost of care into manageable monthly payments.
There’s no special process to pay self-pay. Walk in or check in online ahead of time, let our team know you’ll be paying out of pocket, and your provider will go over any add-on costs before they’re performed — so there are no surprises at checkout.
All-In Bundles: One Price, Visit and Tests Included
Our most popular self-pay option for common conditions is one of our flat-rate bundles — a single price that covers your exam and all relevant tests, so you know your total cost up front.
URI Bundle — $150
For cold and flu symptoms — runny nose, sore throat, cough, congestion, or body aches. The URI bundle covers your exam plus two to five tests based on your symptoms, which may include COVID-19, flu, strep, RSV, and mono. No need to decide which tests you need; your provider picks the right combination based on what’s going on.
UTI Bundle — $150
For urinary symptoms such as burning, urgency, or frequency. The UTI bundle covers your exam, a pregnancy test, a urinalysis, and a urine culture — everything needed to confirm a UTI, rule out pregnancy-related factors, and identify the right treatment in one visit.
STD Testing Bundle — $175
For patients who want comprehensive STD screening in a single visit. This bundle covers all five standard tests from Midwest Express Clinic Labs: chlamydia, gonorrhea, trichomonas, HIV, and syphilis — using both urine and blood samples. No separate office visit fee, no add-ons required for the full standard panel.
Standard Visit + Tests
For illnesses and injuries that don’t fit neatly into one of the bundles above, a standard office visit is $125 and covers your exam and provider evaluation. Additional testing or treatments are billed separately at transparent, flat add-on rates:
- Rapid tests (pregnancy, urinalysis, etc.) — +$10 to $100 depending on the test
- Blood work (CBC, A1C, TSH, etc.) — +$10 to $25
- Ear wax removal — +$35
- Nebulizer treatment — +$40
- IV hydration — +$75
- EKG — +$75
Your provider will review any recommended add-ons and their costs with you before proceeding, so you’re always in control of what you’re authorizing.
Sexual Health & GYN Testing
GU/GYN testing is priced as a flat rate with no additional office visit fee on top. Here’s what’s available:
- STD panel — urine only (chlamydia, gonorrhea, trichomoniasis) — $150
- STD panel — blood only (HIV, syphilis) — $150
- Vaginitis panel (vaginal discharge, itching, or odor) — from $200
- Herpes (HSV) test for a sore or blister — $175
- Office visit (exam included) — $125
If you need a broader range of testing, including site swabs or additional STD services, visit the STD Testing page for the full menu of available tests. Patients seeking ongoing HIV prevention can also ask about PrEP treatment during their visit.
Physicals and Wellness
All physicals and wellness exams are flat-rate with no additional office visit fee:
- School, sports, or camp physical (Pre-K through grade 12) — $30
- College or employment physical — $75
- DOT exam — $125
- Annual wellness exam with labs (UA, CBC, CMP, lipids, TSH, HgA1C; PSA for men) — $125
- TDAP / whooping cough vaccine (ages 7+) — $75
- TB skin test — $60
- 2-step TB test — $120
- TB Gold Blood Test — $120
- Outside injection administration (requires licensed pharmacy order) — $45
- Lab kit assistance (hair, blood, urine, or finger stick) — $45
School physicals include completion of one required form. Additional forms are $10 each. Parents or legal guardians must be present for all patients under 18.
Shots and Services
The following are available as flat-rate walk-in services with no additional office visit fee:
- Vitamin B-12 shot — $10
- Lipo-B or Tri-Immune shot — $25
- Flu shot / immunization (ages 6 months and up) — $35
- 6-panel drug test — $45
- 12-panel drug test — $60
Lipo-B injections are a walk-in wellness service — no prescription or appointment needed. The Tri-Immune Boost is also available at this price point. Drug testing includes specimen validity testing.
Procedures and Imaging
The following procedure and imaging services are available at select Midwest Express Clinic locations. An office visit and provider order from a Midwest Express Clinic licensed provider is required:
- Minor procedures — from $225
- Major procedures — from $275
- X-ray — $50; additional X-ray sets are $25 each
- Ultrasound — $120 (outside provider orders accepted)
Final cost for procedures depends on the complexity of your diagnosis. Your provider will give you an estimate before any procedure begins. Self-Pay vs. Using Your Insurance
Even if you have insurance, self-pay can sometimes be the more practical option — particularly if you have a high-deductible plan and haven’t yet met your deductible for the year. In these situations, our self-pay rates may be lower than what you’d pay out of pocket through your insurance plan before coverage kicks in.
One important note: if your visit is billed through insurance, you are not eligible for self-pay pricing for that visit. Self-pay pricing applies to patients paying at the time of service without insurance billing. If you do have insurance and want to use it, we’re happy to bill your plan directly — visit the Accepted Insurance page for a full list of plans we work with.
Payment Options
We accept cash, debit cards, and major credit cards at every location. CareCredit financing is also available for qualifying patients who prefer to divide their costs into monthly payments — a helpful option for procedures or unexpected visits.
After your visit, you can pay your bill online at any time through our secure patient payment portal.

Prices are for self-pay patients only and apply when payment is made at the time of service. If your visit is billed through insurance, self-pay pricing does not apply. This page does not constitute a price quote or guarantee of final cost — confirm current pricing with your nearest Midwest Express Clinic location before your visit.
Frequently Asked Questions
Do I need insurance to be seen at Midwest Express Clinic?
No. Walk in any day of the week, with or without insurance. Our transparent self-pay pricing makes it easy to understand your costs before you’re seen.
What's the difference between a bundle and a standard visit?
Bundles are flat-rate packages for the most common conditions — your exam and all relevant tests are included in one price. A standard office visit covers your exam only, with add-on tests billed separately. For many patients, a bundle is the lower-cost option because it avoids per-test add-on charges for the most commonly needed combination of tests.
Will I know my total cost before any treatment begins?
Yes. All self-pay pricing is listed upfront, and your provider will review any recommended add-ons and their costs with you before proceeding. Our team can also provide a detailed breakdown of any charges associated with your visit at any time.
Are these prices the same at every location?
Pricing is consistent across Midwest Express Clinic locations. Some services — such as X-rays, ultrasounds, and certain procedures — are only available at locations that have the necessary equipment and staff. Call ahead or check your location’s page to confirm specific service availability.
What if I have insurance but want to pay self-pay instead?
If you’d prefer to use self-pay pricing rather than billing your insurance, that’s your choice to make. Keep in mind that once a visit is billed through insurance, self-pay pricing no longer applies to that visit. Let our front desk know your preference at check-in.
What payment methods do you accept?
We accept cash, debit cards, and major credit cards at all locations. CareCredit financing is also available for qualifying patients. After your visit, bills can be paid online through our secure payment portal.
Can I get an estimate before I come in?
Our self-pay pricing list on this page gives you a strong sense of cost for the most common services. For procedures with variable costs, your provider will give you an estimate before beginning. You can also call your nearest location to discuss pricing for a specific service before your visit.
