What to Bring to Your DOT Physical

Coming prepared helps your visit go smoothly and reduces the chance of needing to return. Bring the following to your appointment:

• A valid government-issued photo ID (driver’s license or passport)
• Your current Medical Examiner’s Certificate, if you have one
• A complete list of current medications, including dosages and the prescribing provider’s name
• Documentation of any known medical conditions — letters from your treating physician are especially helpful for conditions like sleep apnea, hypertension, diabetes, or heart disease
• Glasses, contact lenses, or hearing aids if you use them — you must bring these to the exam
• Records of any recent surgeries or hospitalizations

If you have been treated for insulin-dependent diabetes mellitus and are seeking an exemption to operate a CMV, your treating provider will need to complete the FMCSA MCSA-5870 form. You can download that form directly from the FMCSA website or through our DOT physical page.

How Much Does a DOT Physical Cost at Midwest Express Clinic?

DOT physicals at Midwest Express Clinic are $125. Because DOT physicals are classified as occupational exams rather than medical treatment, they are generally not covered by health insurance. Payment is due at the time of service.

Please note: the exam fee is charged regardless of the outcome. If you are found medically unqualified during the examination, the fee is non-refundable. If a disqualifying condition is identified, your provider can discuss what steps — such as treating a condition or obtaining a waiver through the FMCSA — may allow you to become qualified in the future.

Common Reasons Drivers Are Disqualified — and What You Can Do

Failing a DOT physical doesn’t always mean a permanent disqualification. Many conditions can be managed or documented in a way that still allows certification. Common reasons drivers do not pass include:

• Uncontrolled high blood pressure — treatment and documentation of controlled readings may resolve this
• Cardiovascular conditions — a cardiologist letter may be required
• Untreated or poorly managed sleep apnea — treatment compliance documentation is key
• Vision that does not meet FMCSA standards without correction — corrective lenses must be worn while driving
• Hearing that does not meet standards without correction — hearing aids are permitted
• Diabetes requiring insulin — the FMCSA insulin exemption program may apply; the MCSA-5870 form is required
• Recent substance use or a history of drug or alcohol-related offenses

If you have a medical condition you are concerned about, bring documentation from your treating provider. Our medical examiners review all submitted records as part of the evaluation and will tell you clearly what, if anything, needs to be addressed before a certificate can be issued.

How Often Do I Need a DOT Physical?

Most commercial drivers are required to renew their DOT medical certification every two years. However, if you have a medical condition that requires monitoring — such as hypertension or diabetes — your medical examiner may issue a shorter certification period (for example, one year or six months) to allow for follow-up before the next renewal. The certification period is determined by your medical examiner based on your individual health profile.
