Midwest Express Clinic offers multiple ways to view your patient information and test results through our online portal and MyQuest app. For detailed instructions on setting up your account and access your information, please view the instructions below.

Midwest Express Clinic Patient Portal


For assistance using the Midwest Express Clinic online portal and accessing your test results, please follow the instructions below.

Patient Portal - Instructions

  • You will receive an email from [email protected].
    • If you did not receive an email, please check your spam or junk folder.
  • To create your account, use the “Click Here” highlighted at the bottom of the email.
    • Verify Your New Account: Click Here.
  • You will then be directed the Secure Patient Portal. Your username will be the email address you gave to the clinic during registration.
  • Enter the patient’s Date of Birth.
  • Create and confirm your password.
    • You can change your password or add an alternative email by going to the My Account tab at the top of the page.
  • To view your results, go to the Inbox tab and highlight the test results you would like to view.
    • You can then click on the paperclip (left of the clinic name) to open the message.
    • Click download to view the document.

Forgot Your Password?

If you need help accessing the Midwest Express Clinic patient portal or forgot your password, you may follow the instructions below to reset your account.

Forgot Your Password - Instructions

  • Visit the secure Patient Portal.
  • Click on Forget Password?
  • Click on the Patient User radio button.
  • Enter Username.
    • Your username is the email address provided to clinic at time of registration.
  • Enter Date of Birth.
    • If you have multiple dependents use the dependent’s date if birth that you used to initially sign into the portal.
  • You will receive an email from [email protected]. The email will prompt you to click on a link to reset your password.
    • If you did not receive an email, please check your spam or junk folder.
  • Once you click the link, you will be sent to the Secure Patient Portal to reset your password.
    • Confirm your new password.
    • Enter your new password.
  • Finally, return to the login page where you will need to sign in with your new password.
