DOT Physicals

A Department of Transportation (DOT) physical examination must be conducted by a licensed medical examiner listed on the Federal Motor Carrier Safety Administration (FMCSA) National Registry. At Midwest Express Clinic, our providers are registered with the FMCSA to perform these examinations.

If the medical examiner determines that the individual is physically qualified to drive a commercial motor vehicle (CMV), they will provide a copy of the results to the individual and complete a Medical Examiner’s Certificate.

What to expect during the DOT Physical appointment?

The exam usually takes about 30 to 45 minutes. During the examination, the provider will review your medical history and perform some tests to ensure you’re in good health to work safely.

The exam includes:

  • Vision and hearing tests
  • Blood pressure and pulse rate check
  • Urinalysis to check for underlying medical conditions
  • Physical examination (checking heart, lungs, abdomen, neurological function, and more)

What to bring to the appointment?

  • A valid photo ID (e.g., driver’s license)
  • Your current medical certificate (if applicable)
  • List of current medications, including dosage and prescribing doctor’s name
  • Information about any health conditions and recent medical procedures or surgeries
  • Vision or hearing aids, if applicable
  • Glasses or contact lenses, if applicable

Remember, this is the medical examination portion required for your license. You will still need to take the completed paperwork to the Bureau of Motor Vehicles (BMV) to apply for licensure.

Common reasons for disqualification include uncontrolled high blood pressure, heart conditions, severe sleep apnea, or drug and alcohol use. Please note that, unfortunately, we are unable to offer refunds regardless of the exam’s outcome.

What if I fail my DOT Physical?

If you fail your DOT physical, you can seek treatment for the disqualifying condition and reapply. Some conditions may have specific waiver or exemption programs through the FMCSA.

What if I have a medical condition?
