1. Eligibility:
• The sweepstakes is open to legal residents of the United States of America, aged 18 or older at the time of entry.
• Employees of Midwest Express Clinic and their immediate family members are not eligible to participate.
2. How to Enter:

• Tag two friends in the comments of the sweepstakes post.

• Tag two friends in the comments of the sweepstakes post.
• Alternative Method of Entry (AMOE):
• To enter without liking, following, or tagging, send a 3×5 inch card with your name, address, phone number, and email address to: 1520 North Damen Ave, Chicago, IL 60622
• Entries must be postmarked by 8/6/2024 and received by 8/26/2024.
3. Entry Period:
• The first sweepstakes begins on 8/6/2024 and ends on 8/12/2024 at 11:59 PM CT
• The second sweepstakes begins on 8/13/2024 and ends on 8/19/2024 at 11:59 PM CT
• The third sweepstakes begins on 8/20/2024 and ends on 8/26/2024 at 11:59 PM CT
4. Prizes:
• Three (3) winners will receive autographed merchandise from Coby White.
• The prize is non-transferable, and no cash or other substitution will be made except as provided herein at the Sponsor’s sole discretion.
5. Winner Selection and Notification:
• The first winner will be selected in a random drawing conducted on or about 8/13/2024.
• The second winner will be selected in a random drawing conducted on or about 8/20/2024.
• The third winner will be selected in a random drawing conducted on or about 8/26/2024.
• The winners will be notified via Instagram/TikTok Direct Message or email and must respond within 48 hours to claim the prize.
• If the winners do not respond within 48 hours, an alternate winner will be selected.
6. General Conditions:
• By participating, entrants agree to be bound by these Official Rules and the decisions of the Sponsor, which are final and binding in all respects.
• The Sponsor reserves the right to cancel, suspend, and/or modify the sweepstakes if any fraud, technical failures, or other factors impair the integrity or proper functioning of the sweepstakes, as determined by the Sponsor in its sole discretion.
