For assistance creating an account for the Midwest Express Clinic online portal, please follow the instructions below.
Creating an Account – Instructions
- At the end of your visit, you will receive a PIN to create your online account. Hold on to this information as you will need it later.
- Visit the secure Patient Portal.
- Click on Create an account.
- Enter your email address and choose a password.
- Select Yes for the question Did your provider give you a PIN?
IMPORTANT: The PIN will be sent via email. If you did not receive it, please contact the Patient Portal Help Line at (630) 296-5323. - Choose an option for the patient you’re registering (yourself or child/dependent).
- Enter the PIN and the patient’s birthday.
- Set up a security question, agree to the terms and conditions, then click on Create my account.

Patient Portal — Watch Below
The Patient Portal is where you can view your complete lab history and past medical information over time. Use it to track trends, download previous results, and stay connected to your ongoing health records.
CareAgent — Watch Below
CareAgent is how you’ll receive important visit information after your appointment. This includes your discharge instructions, lab results, and follow-up details — delivered directly to you for easy access. It keeps you informed about your most recent visit and next steps in your care.
Please note: If multiple labs were ordered, you may receive separate text messages as each result becomes available. This is normal and ensures you receive your results as soon as they’re ready.
